Referral Pathways

The following pathways provide information for primary care and secondary care colleagues in Queen Elizabeth, Queen Mary, and Lewisham hospitals about when and where to refer patients to vascular services. The Supervised exercise referral pathway (claudication patients) is also detailed below.

Referral Forms
