The Cardiac ODN Improving Referrals workstream has updated the King’s Health Partners referral form for open access echocardiography. Streamlined and consistent, the new form offers simple check boxes of symptoms and notes for referral reasons.

The form is downloadable from the following sites, and has been sent to CCGs for upload to South East London GP systems.
